TSU CAT 2021 Advisory No. 2

Submission of application and requirements for TSU CAT 2021 will start on January 25, 2021.

Please prepare in advance the following:

1.  Active personal email account (for online application)
2.  Digital image/scanned passport-sized (1.77x1.37) photo with name tag (DELA CRUZ, JUANA CRUZ) and white background
3.  Digital image/scanned copy of PSA Birth Certificate (front page)
4.  Digital image/scanned copy of Certificate of Good Moral Character (front page)
5.  Digital image/scanned copy of Report Cards/ALS Certification:
   1.  For currently enrolled Grade 12 students(S.Y. 2020-2021)
      • Digital image/scanned copy of Form 138 Grade 11 (Report Card) with GWA (front and back page)
      • Digital image/scanned copy of Form 138 Grade 12 (Report Card) with at least first quarter GWA (front and back page)
   2.  For SHS graduates(graduated last 2018-2020)
      • Digital image/scanned copy of Form 138 Grade 12 (Report Card) with GWA (front and back page)
   3.  For Non-SHS graduates(graduates of old high school curriculum)
      • Digital image/scanned copy of Form 138 (Report Card) with GWA (front and back page)
   4.  For ALS graduates(Alternative Learning System Graduates Eligible for College Admission)
      • Digital image/scanned copy of ALS Certification with rating (front page)
